position is located in Salem, however this is dependent on the
successful candidate and does have the ability to be located out of
another Department of Corrections location within the state.About
the Job Your RoleThe primary purpose of this position is to expand
on our clinical pharmacy services offered to our adults-in-custody.
This includes development, design, implementation, and monitoring
of all existing and future Collaborative Drug Treatment Management
(CDTM) programs and Collaborative Practice Agreements (CPA).This
position is also tasked with managing drug information services to
our practitioners. This includes organizing and facilitating the
Pharmacy & Therapeutics Committee, accepting requests for formulary
inclusion and exclusions, reviewing effectiveness and safety of
medications, and participating or supervising clinical pharmacists
involved in Therapeutic Level of Care Committees.Pharmacists
involved in MTM services may develop a medication treatment plan
for monitoring and evaluating the patients response to therapy, as
well as monitoring and evaluating the safety and effectiveness of
medication therapy.Provide education and training to the patient or
the patients agent on the use or administration of a medication.
This position is responsible for supervising the Clinical
Pharmacist associated with MTM services.There are also
opportunities to provide direct patient care where applicable.This
position is management services and may be eligible for Police &
Fire (P&F) benefits through Public Employees Retirement System
(PERS), as defined by statute.Minimum QualificationsA current
Pharmacist's license issued by the Oregon State Board of Pharmacy
and one year of experience providing pharmaceutical
services.Special QualificationsGraduation from an accredited School
of Pharmacy and registration as a Pharmacist by the Oregon State
Board of Pharmacy.You must have a valid drivers license and a good
driving record or be able to provide an acceptable alternative
