Job Description:

Job Summary: This position is a licensed practical/vocational nurse role designed to provide direct nursing care to patients in a post- acute care setting in a contract capacity. LVN will work closely with other permanent and temporary staff and assist in their duties as needed.


Essential Job Duties:

• 
  
• Contributes to the assessment of patients by collecting data for analysis by an RN.
  
• Performs plan of care interventions, including specimen collection and wound care.
  
• Demonstrates proper infection control practice and adheres to Infection Control Policies and Procedures and OSHA standards.
  
• Adheres to medication pass protocol and meets regulatory guidelines.
  
• Evaluates nursing care provided by continual review of the effectiveness of the nursing care plan.
  
• Collaborates with supervising nurse (when present) to make appropriate revisions to achieve the desired outcomes.
  
• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  
• Works as an advocate for the physical and emotional well-being of the patient.
  
• Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
  
• Collects and documents patient information as needed (including weights and vital signs).
  
• Provides patient and family education in regard to the hospitalization/admission, medications, and/or in preparation for discharge.
  
• Upon discharge, provides appropriate discharge instructions to include follow-up and referral needs.
  
• Provides accurate and timely hand-off communication.
  
• Serves as clinical and administrative resource to staff, residents, and families regarding nursing services and procedures, aging processes, medications, and outside medical services.
  
• Supervises CNA's and Caregivers.

      Job Requirements Education/Experience:
      
      • 
        
      • Qualified candidates will have graduated from an approved Licensed Practical Nurse program.
        
      • Must possess a current, unencumbered license to practice as a LPN in this state.
        
      • Licensure/Certifications: Current LPN licensed in good standing by their State Board of Nursing.
        
      • Current BLS or CPR/First Aid; Food Handlers Card.

        • Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when a patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other team member, when available.
          
        • Unique patient lifting/movement situations will be assessed on a case-by-case basis.
